GENETIC COUNSELING training PROGRAM AGREEMENT BETWEEN DUKE UNIVERSITY HEALTH SYSTEM AND ACADEMIA SINICA This agreement is effective July 1, 2002 to June 31, 2005 WITNESSETH Whereas, Duke desires to provide a clinical setting for trainees to train, experience and enhance their skills in genetic counseling and genetic research, and Whereas, Academia Sinica wishes to send certain trainees to Duke for additional training in genetic counseling and genetic research, NOW THEREFORE, in consideration of the covenants set forth, Duke and Academia Sinica agree as follows: Duke's Responsibilities: Duke agrees to provide clinical and research facilities suitable for the fulfillment of the objectives of the course of study (see Exhibit A). Duke agrees to accept a maximum of 18 trainees from the Academia Sinica Genetic Counseling Program over the course of the 3 years of the contract. Duke will provide a coordinator to regularly communicate with Academia Sinica regarding numbers of trainees, rotations, evaluations, and all other details to effectuate the smooth operation of the program. The trainees will be fully supervised in all educational, observational activities by a trained genetic counselor (board certified or active candidate status) and/or a medical geneticist or attending physician. Each Academia Sinica Genetic Counseling trainee will receive training and complete the schedule as outlined in Exhibit A. Under the guidance and supervision of on-site clinical geneticists, genetic counselors, and or attending physicians, trainees will study the activities described in section 8 of Academia Sinica00 Understandings and Responsibilities. Duke agrees that all patient care activities are the responsibility of Duke. Duke shall have the right to exclude a trainee or faculty member from the Hospital in the event that Duke determines that individual is not performing satisfactorily or is interfering with the Hospital's operations; provided, however, that the person has been made aware of the intent to release him/her and has been given the opportunity to respond prior to the release. In the event that Duke elects to exclude a trainee or faculty member, it shall immediately notify Academia Sinica. Duke agrees to provide suitable classroom, conference and other teaching space adequate for the trainee00 experience. Duke will require Academia Sinica00 trainees to participate in orientation sessions to familiarize themselves with Duke clinical setting and complete appropriate clinical credentialing and safety training to perform tasks in this setting. Duke agrees to share all appropriate information with Academia Sinica regarding both patients and services in order for the trainees to perform their professional services. Trainees understand patient's rights to confidentiality and all information shared with trainee and Academia Sinica are held in confidence. Duke will permit the use of its libraries and cafeteria by the trainees at their own expense. Duke will provide first aid for injuries to the trainees on the hospital premises, but the trainees are responsible for all costs.
Academia Sinica's Understandings and Responsibilities: Academia Sinica understands, that as visiting trainees, the trainees may participate in verbal interaction with patients, however, they will not have clinical responsibility. Academia Sinica understands that the trainees do not have any clinical privileges and will not engage in any type of clinical activity while at Duke University Health System. Academia Sinica also understands that in the event that its trainees are requested to participate in any clinical activity that its trainees must decline. All parties understand that this program is an educational experience and that it will not result in genetic counselors that meet the certification requirements of the American Board of Genetic Counselors. Academia Sinica will provide accident and health insurance for its trainees and will ensure that each trainee has appropriate insurance prior to the beginning of this educational experience. In consideration for the trainees being allowed to participate in this activity, Academia Sinica hereby acknowledges and agrees that its trainees will in no way copy or preserves by paper writing, electronic or by any other means any patient specific information nor any patient identifying information. Further, Academia Sinica acknowledges and agrees that its trainees will not communicate nor discuss any patient specific information with anyone except those involved with learning activities that are also employees or faculty members of Duke University Health System. Academia Sinica and its trainees promise and agree to keep confidential all patient information and to respect the privacy of the patients at Duke University Health System. Academia Sinica and its trainees make these promises and representations freely and voluntarily and understand that others are acting in reliance on them. Academia Sinica agrees that the trainees are not agents, servants, or employees of Duke, and are present at Duke solely as trainees of Academia Sinica. Academia Sinica agrees that the trainee00 experience is educational and will at all times be directly supervised by program personnel. This observational educational experience will involve studying the processes involved in: Gathering medical and family history information. Collecting pedigree data for genetic linkage and research studies Discussion of genetic risk assessment based on family history and other factors such as age or ethnicity. Diagnosis and management of genetic diseases Patient education about genetic or possibly genetic conditions. Identifying resources (educational and support) for patients and their families. 9. Academia Sinica will appoint a coordinator to act as a liaison with Duke's coordinator to arrange for rotations and other details to effectuate the goals of the program. 10. Academia Sinica agrees to submit a letter to the coordinator every 6 months specifying names and number of trainees; and dates the trainees will be at Duke University Medical Center. 11. Trainees will attend required safety training prior to their clinical experience. 12. Academia Sinica will ensure that each trainee has received appropriate immunizations prior to the beginning of any clinical rotation at Duke (Attachment A). 13. Academia Sinica agrees to sponsor this genetic counseling trainee program. Academica Sinica agrees to commit to the funding of this genetic counseling trainee program to the Division of Medical Genetics, the Department of Pediatrics the amount detailed in Exhibit B. Miscellaneous: 1. This agreement may be terminated by either party by giving the other 30 days written notice of the intent to terminate. Each party may, in its discretion, permit the completion of any trainee's experience through the termination date. 2. The agreement may be modified or amended by mutual consent of both parties as long as the amendment is executed in the same fashion as this agreement. 3. This agreement will be automatically renewed for the terms of the contract unless either party gives the other 30 days written notice of the intent to terminate. 4. The agreement shall be interpreted and construed in accordance with the laws of North Carolina.

Exhibit A The following is an outline of the training schedule for the Academia Sinica Genetic Counseling trainees. The trainees will be grouped into groups of 3 for duration of 5 months training at Duke University Medical Center. This schedule will allow training of a maximum of 18 trainees in three years. Duration 4 weeks Short Course and Orientation to Medical Genetics at Duke The objective of the short course and the orientation to Medical Genetics at Duke is to ensure that the trainee is able to maximize their clinical experiences. The short course will focus on the development of skills required for clinical and research genetic counseling. Examples of areas that will be addressed include: pedigree taking and analysis; genetic counseling skills, theories, and techniques; clinical presentations of cases, enrolling subjects in research studies, research ethics, the Institutional Review Board, clinical trials, and the roles of study coordinators. 6 weeks Medical Genetics Trainees will be exposed to a wide variety of types of patients the Medical Genetics Clinic. The types of patients seen in this clinic include: dysmorphic syndromes, presymptomatic testing for adult onset disorders, neurofibromatosis, and Marfan syndrome. Additionally, the trainee will have the opportunity to participate in the specialty clinics for Chromosome disorders (Down syndrome) and the Metabolic Clinic. Clinical Trials: As part of their rotation in Medical Genetics the trainees will be exposed to the components of clinical trials. 6 weeks Center for Human Genetics This rotation will provide the trainees with exposure to the collection of pedigree data for research purposes. Additionally, the genetic counselors in the Center for Human Genetics are actively involved in collecting data from families enrolled in linkage analysis. This rotation will provide the trainees with basic knowledge on ethics and family dynamics needed to enroll human subjects in genetic linkage studies. 2 weeks Prenatal Genetics The prenatal rotation will provide the trainees with exposure to genetic counseling and prenatal diagnosis in a tertiary care center. This will include: genetic counseling for prenatally diagnosed disorders and advanced maternal age and observation of amniocentesis and level III ultrasound. 2 weeks Cancer Genetics This rotation will allow the trainees to be exposed to genetic counseling for breast and ovarian cancer, risk models for cancer counseling, and epidemiology.
