The attached documents are provided to assist a covered entity in establishing a training process for their workforce members.  The documents available for your customization include: 

Exhibit 2 - HIPAA Privacy Training Process  

This exhibit explains the different functions required to establish a process to train workforce member about basic HIPAA privacy rules that may impact their jobs.  It provides information on: 

Which organization is required to provide training to their workforce members Who within the organization must be trained When the workforce members must be trained Who may provide the training What HIPAA information must be covered in the training What documentation is required to be maintained about the training Decision Points on where to make policy decisions about this process  
Exhibit 3 00HIPAA Privacy Sanctions Process  

This exhibit presents the factors to consider when setting up a sanction process to be used when members of a covered entity00 workforce violate HIPAA privacy rules.  It provides information on: 

Who Is and Is Not Covered Documentation Sanctions vs. Reasonable Safeguards Training and Certification Existing Disciplinary Procedures Mitigation  
Exhibit 4 00Basic HIPAA Privacy Training Package  

This exhibit is a PowerPoint training package.  It is divided into ten components so that the training can be provided at different times.  The components include: 

Introduction Covered Entities Protected Health Information Your Rights Privacy Officer Access Use and Disclosure of Protected Health Information Safeguards Complaints Sanctions  

The slides include notes the trainers will need to provide during the training.  In addition, decision points are included in the red boxes and State law impacts in the blue boxes.  Covered entities will need to analyze the decision points and federal and State laws and regulations and insert their Privacy Policies and Procedures into the training presentation. 

Exhibit 5 00HIPAA Privacy Basic Training Instructions and Conversion Tool  

This exhibit provides slide-by-slide instructions on what the slide is intended to cover and how to convert the slide to reflect the organization00 Privacy Policies and Procedures.  The instructions indicate: 

Which slides are optional Where policies need to be added Where State law effects the information provided on the slide  
Exhibit 6 00HIPAA Privacy Basic Training Glossary  

This exhibit is a glossary of the HIPAA terms that are introduced in the HIPAA Privacy Basic Training PowerPoint presentation.  This glossary is not all inclusive of HIPAA privacy terms, but specific to those required for the training. 

Exhibit 7 00Employee Privacy Basic Training Acknowledgement  

This exhibit is an optional tool that may be used to enhance the importance of understanding the HIPAA training by members of a covered entity00 workforce.  It provides a form that members may sign that states that they understand the HIPAA privacy training that they have taken. 

Exhibit 8 00Certificate of Attendance at Basic HIPAA Privacy Training  

This exhibit is a sample certificate of attendance that may be customized and provided to workforce members who attend basic HIPAA privacy training. 

Exhibit 9 - Attendance Tracking Tool  

This exhibit is a manual tracking tool that may be used to track who attends the training and on which components they receive training and when these trainings took place. 

Exhibit 10 - Federal and State Law Citations  

This exhibit document provides the text of the federal regulations and State law cited in the Training and Sanction Process package.
