February 26, 2007 SUBJECT: RFP NO. 37 06 Technical Assistance and Consultant Services Dear Prospective Bidder:
You are invited to submit a proposal for the above subject RFP for the Commonwealth of
Pennsylvania, Department of Public Welfare in accordance with the attached Request for
Proposal (RFP). This RFP is being issued contingent upon federal approval and is subject to
change resulting from their review. DPW will issue such changes as an addendum to the RFP.
All proposals must be submitted in the following manner to the Pennsylvania Department of Public
Welfare, Bureau of Contract Policy, Management and Procurement, Room 106, Health and Welfare
Building, Commonwealth Avenue & Forster Street, Harrisburg, Pennsylvania 17120.
one (1) original and ten (10) paper copies of the Technical Submittal one (1) original and ten (10) paper copies of the Cost Submittal one (1) original and one (1) paper copy of the Disadvantaged Business Submittal one (1) original and one (1) paper copy of the Contractor Partnership Program
Submittal one (1) original and one (1) paper copy of the Domestic Workforce Utilization
Certification (separately sealed and included in the original cost submittal envelope)
and one (1) complete and exact copy of the technical proposal on CD-ROM in
Microsoft Office or Microsoft Office-compatible format.
Proposals must be received at the above address no later than two oclock p.m. (2:00
P.M.) on March 30, 2007. Late proposals will not be considered regardless of the reason.
A Preproposal Conference will be held at 1:00 p.m. on March 13, 2007 in Room 505, Health
and Welfare Building, Commonwealth Avenue & Forster Street, Harrisburg, Pennsylvania
17120. All questions should be directed via email, (with the subject line RFP 37-06
Question) to Ms. Darlene DeMore, Director, Division of Program Development and Support,
Bureau of Managed care Operations, at ddemore@state.pa.us no later than March 9, 2007. Contractors should not attempt to contact the RFP Project Officer by any other means.
Proposals must be signed by an official authorized to bind the vendor to its provisions. Also,
please be sure to include your Federal Identification Number, SAP Vendor ID Number and the
Point of Contacts e-mail address on Appendix L, Proposal Cover Sheet. Evaluation of
proposals and selection of vendors will be completed as quickly as possible as receipt of
