The following actions have been taken by the Division following hearings conducted before an Administrative Law Judge and
the respective Licensing Committees or Boards or have been entered by Stipulation from Oct. 1, 2005 Dec. 31, 2005.

RESPONDENT/ADDRESS ACTION
PHYSICIANS/SURGEONS
ANDREW, Larry Glen, DO Cause of Springville, Utah Action: Respondent's continued practice represents an immediate and
significant danger to the public health, safety and welfare. Emergency
Order: Respondent's licenses to practice as an osteopathic
physician/surgeon and to administer and prescribe controlled
substances were immediately suspended pending a further order of
the Division. Date: December 23, 2005
BAILEY, Stanton Anderson, MD Amended Santaquin, Utah Order: The probationary condition requiring Respondent to attend 12-step
program meetings at least five (5) times per week was changed to
require Respondent to attend a minimum of four (4) meetings per
week. Date: November 3, 2005
BERK, Howard Sidney, MD Cause of Murray, Utah Action: Respondent engaged in behavior in July 2005 that may adversely
reflect on his ability to safely and competently practice as a
physician/surgeon. Order: Respondent's licenses to practice as a physician/surgeon and to
administer and prescribe controlled substances are temporarily
restricted to the non-clinical practice of medicine. Respondent shall
also restrict his practice to: (a) paper review of care given by other
medical practitioners, (b) research, and (c) education. Respondent
shall not engage in direct patient contact, interview, examination,
diagnosis, treatment, care or prescription of any kind. Date: November 7, 2005
CLARK, Jeffery Allen, MD Cause of Provo, Utah Action: Respondent has had a lengthy history of substance abuse problems
and controlled substance violations. Order: Respondent's license to administer and prescribe controlled
substances was revoked effective the date of the Order.
Respondent's license to practice as a physician/surgeon was
revoked with a stay of enforcement on the revocation. Said
physician/surgeon license is suspended until certain conditions
have been met. Following the period of suspension, said license

GUERNSEY, Bruce A., DO Cause of Salt Lake City, Utah Action: Respondent has a pending federal criminal charge filed against
him. Order: Respondent's licenses to practice as an osteopathic
physician/surgeon and to administer and prescribe controlled
substances are temporarily restricted until a further order is issued
by the Division. Date: October 3, 2005
PACK, Steven Richard, MD Cause of Alpine, Utah Action: Respondent is aware the Division is conducting an investigation
relative to his prescribing of controlled substances. Order: Respondent voluntarily surrendered his license to prescribe and
administer controlled substances until such time as the current
Division investigation is resolved. Date: December 2, 2005
PROSKAUER, Stephen, MD Reinstatement Salt Lake City, Utah Order: The probation on Respondent's licenses to practice as a
physician/surgeon and to administer and prescribe controlled
substances was terminated and said licenses were reinstated with
full privileges. Date: December 20, 2005
WEBER, Nita Beth, DO Amended Nephi, Utah Order: The suspension on Respondent's license to administer and
prescribe controlled substances was terminated and said license
was placed on probation subject to the same probationary
conditions that currently exist on her license to practice as an
osteopathic physician/surgeon. Date: December 22, 2005
DENTISTS
FIFIELD, Perry Melvin Cause of Clearfield, Utah Action: In October 2003 and April 2004 Respondent obtained and provided
protected information belonging to D.W. contained in the Utah
Controlled Substance Database to his employer who was involved
in an ongoing domestic dispute with D.W. In April 2003
Respondent was disciplined by the California Dental Board. When
Respondent renewed his Utah dental licenses in May 2004, he
failed to notify the Division of the disciplinary action taken in
California. Order: Respondent's licenses to practice as a dentist and to administer
and prescribe controlled substances are revoked with a stay of
enforcement on the revocation. Said licenses are placed on

DENTISTS - continued
FORD, Kraig R. Cause of Heber City, Utah Action: Respondent failed to make sure controls were in place regarding
the maintenance of his office's nitrous oxide supply. Respondent
also had previously prescribed a Schedule III controlled substance
to a patient for lower gastrointestinal problems, a condition which
Respondent is not licensed to treat. Respondent also failed to
properly document controlled substance prescriptions issued to
patients in patient charts on numerous occasions. Order: Respondent's license to practice as a dentist with a Class II
anesthesia permit was revoked. A license to practice as a dentist
with a Class I anesthesia was issued to Respondent. Respondent's
licenses to practice as a dentist with a Class I anesthesia permit
and to administer and prescribe controlled substances were
revoked with a stay of enforcement on the revocation. Said licenses
were placed on probation for two years with conditions. Respondent was also ordered to pay a $1,000 fine within 90 days. Date: October 26, 2005
REGISTERED NURSES/ADVANCED PRACTICE NURSES
BAIRD, Jamie Steadman Cause of Tooele, Utah Action: Due to poor health, Respondent is not able to comply with
probationary conditions that exist on her license. Order: Respondent's license to practice as a registered nurse is indefinitely
suspended. Respondent may apply for the termination of the
suspension at any time upon compliance with certain conditions
before the suspension can be terminated. Date: October 18, 2005
BUDGE, Jennifer Lea Cause of Monticello, Utah Action: Respondent did not feel she is able to comply with terms and
conditions that exist on her license. Order: Respondent voluntarily surrendered her license to practice as a
registered nurse. Respondent may not reapply for licensure for a
period of three years. Date: November 14, 2005
CHIPMAN, Brenda Amended American Fork, Utah Order: The probationary condition requiring Respondent to consult with
only one physician with regards to prescriptions was changed to
allow Respondent's primary care physician, Dr. Marshall Willis, to
prescribe all mood altering or narcotic medications and to allow
other practitioners in Dr. Willis practice to prescription other
medications to Respondent. Date: October 27, 2005
COX, Tiffany Ann Cause of Heber, Utah Action: Respondent's license to practice as a registered nurse expired on
January 31, 2005 yet Respondent continued to practice as a nurse. Order: Respondent was publicly reprimanded and ordered to pay a $500

DAVIS, Jean Bartholomew Cause of Lehi, Utah Action: Respondent continued to practice as a registered nurse after her

GILES, Cheryl Ann Cause of Price, Utah Action: Respondent continued to practice as a registered nurse after her

GUBLER, Joleen M. Amended St. George, Utah Order: The probationary condition restricting Respondent's access to
controlled substances was terminated. Date: October 27, 2005 Amended
Order: The probationary condition requiring Respondent to participate in
therapy and/or aftercare was terminated. Date: November 21, 2005
JEPSON, W. Scott Cause of Provo, Utah Action: Respondent has been engaging in practices which represent an
immediate and significant danger to the public health, safety and
welfare. Emergency
Order: Respondent's license to practice as a registered nurse was
immediately suspended effective the date of the Order pending a
hearing or a further Order of the Division. Date: October 11, 2005
JORGENSON, Crystal Lee Cause of Salt Lake City, Utah Action: Respondent engaged in unprofessional conduct prior to submitting
an application for licensure. Memorandum of Understanding and Order: A license to practice as a registered nurse will be issued to
Respondent upon her successfully passing the NCLEX-RN
examination. Said license will be placed on probation for five years
with conditions and restrictions. Date: November 17, 2005
KRACZEK, Renee Whittaker Cause of Farmington, Utah Action: Respondent continued to practice as a registered nurse after her
license expired on January 31, 2003. Order: Respondent was given a public reprimand and ordered to pay a
$500 fine within 90 days. Date: November 30, 2005 LASSON, Stacey J. Cause of Taylorsville, Utah Action: Respondent has a substance abuse problem that might reasonably
be considered to impair her ability to safely engage in the practice
of nursing. Order: Respondent voluntarily surrendered her license to practice as a
registered nurse. Respondent may not reapply for licensure for a

LOHR, David B. Cause of Ogden, Utah Action: Respondent has an alcohol abuse problem that might reasonably
be considered to impair his ability to safely practice nursing. Order: Respondent voluntarily surrendered his license to practice as a
registered nurse. Respondent may not reapply for a nursing license
for a period of five years from the Order date. Date: November 3, 2005
MERRELL, Stuart Leon Cause of Salt Lake City, Utah Action: Respondent failed to comply with conditions that existed on his
license. Order: Respondent voluntarily surrendered his license to practice as a
registered nurse. Respondent may not reapply for a nursing license
for a period of five years from the Order date. Date: November 2, 2005
NELSON, Scott J. Cause of Salt Lake City, Utah Action: Respondent has failed to comply with probationary conditions that
exist on his license. Due to Respondent's current profession, which
is not nursing, he is unable to comply with all of the probationary
conditions. Order: Respondent shall pay the $1,000 fine which was ordered in the
3-24-2005 Stipulation and Order within 90 days of this Order.
Respondent's license to practice as a registered nurse is indefinitely
suspended. Respondent has conditions to be completed before the
suspension can be terminated. Date: November 3, 2005
OLSON, Nadine K. Reinstatement Provo, Utah Order: The probation on Respondent's license to practice as a registered
nurse was terminated and said license was reinstated with full
privileges. Date: December 16, 2005
ROONEY, Ann Cause of Salt Lake City, Utah Action: Respondent has engaged in conduct, including the use of
intoxicants and controlled substances that might reasonably be
considered to impair her ability to safely engage in the practice of
nursing. Order: Respondent's license to practice as a registered nurse was revoked
with a stay of enforcement on the revocation. Said license was
placed on probation for five years with conditions and restrictions. Date: November 3, 2005
SALAS, Glenda J. Amended North Ogden, Utah Order: The probationary condition restricting Respondent's access to
controlled substances was terminated. Date: December 19, 2005 STEINER, Richard Alan Cause of St. George, Utah Action: Respondent has a substance abuse problem that, if left untreated,
renders him unsafe to practice nursing. Respondent took numerous
units of a Schedule III controlled substance form his employer
without authorization and without a legal prescription. Respondent
later ingested the controlled substances he took. Order: Respondent was ordered to pay a $1,000 fine within 90 days.
Respondent's license to practice as a registered nurse was revoked
with a stay of enforcement on the revocation. Said license was
placed on probation for five years with conditions and restrictions. Date: October 11, 2005 Newsletter - January 1, 2006 - Page 6

STORMS, Dawna P. Cause of Salt Lake City, Utah Action: Respondent has a substance abuse problem that, if left untreated,
may render her unfit to practice nursing. Order: Respondent was ordered to pay a $500 fine. Respondent's license
to practice as a registered nurse was revoked with a stay of
enforcement on the revocation. Said license is suspended until
such time as certain conditions have been completed. Following
the period of suspension, said license will be placed on probation
for five years with conditions and restrictions. Date: December 13, 2005
WHITEBREAD, Teresa L. Cause of Riverton, Utah Action: Respondent has an alcohol abuse problem that might reasonably
be considered to impair her ability to safely engage in the practice
of nursing. Order: Respondent's license to practice as a registered nurse was revoked
with a stay of enforcement on the revocation. Said license was
placed on probation for five years with conditions and restrictions. Date: November 16, 2005 Amended
Order: The probationary condition restricting Respondent's access to
controlled substances was amended to allow Respondent to have
access to controlled substances only while employed in the labor
and delivery section at IHC central urban facilities if she is actively
participating in the Employee Assistance Program. Date: December 22, 2005
WILLDEN, Virginia B. Cause of Tremonton, Utah Action: In May 2005 Respondent knowingly falsified blood sugar levels in
the record of a diabetic patient and failed to document several of the
patient's blood sugar readings. Order: Respondent was given a public reprimand and ordered to pay a
$500 fine within 90 days. Date: November 30, 2005
WILLIAMS, Teresa Yonk Reinstatement Hyrum, Utah Order: The probation on Respondent's license to practice as a registered
nurse was terminated and said license was reinstated with full
privileges. Date: October 6, 2005
LICENSED PRACTICAL NURSES
CAMPBELL, Karen L. Reinstatement Orangeville, Utah Order: The probation on Respondent's license to practice as a licensed
practical nurse was terminated and said license was reinstated with
full privileges. Date: November 21, 2005

EVENSON, Kristine Reinstatement Centerville, Utah Order: The probation on Respondent's license to practice as a licensed
practical nurse was terminated and said license was reinstated with

HANSEN, Kelli Darlington Amended Murray, Utah Order: The suspension on Respondent's license to practice as a licensed
practical nurse was terminated and said license was placed on
probation for five years with conditions and restrictions identified in
the April 26, 2004 Stipulation and Order. Date: November 21, 2005
KNOWLES, Linda Cause of Layton, Utah Action: While employed as a LPN, Respondent conspired with a nursing
aide to provide the aide with a Schedule III controlled substance.
The nursing aide would sell the controlled substance and return the
proceeds to Respondent. Respondent also provided the aide with
the controlled substance for the aide's personal use. Order: Respondent voluntarily surrendered her license to practice as a
licensed practical nurse. Respondent may not reapply for licensure
for a period of five years. Date: November 8, 2005
KOELLER, Bambi Bodenhofer Amended Midvale, Utah Order: The suspension on Respondent's license to practice as a licensed
practical nurse was terminated and said license was placed on
probation for five years subject to conditions and restrictions
identified in the Division's September 22, 2003 Memorandum of
Understanding and the December 22, 2005 Amended Order. Date: December 22, 2005
MARTINEZ, Barbara A. Cause of Salt Lake City, Utah Action: Respondent's license to practice as a licensed practical nurse
expired on January 31, 2004 yet Respondent continued to work as
a nurse until October 2005. Order: Respondent was publicly reprimanded and ordered to pay a $500
fine within 90 days of the Order date. Date: November 18, 2005
WALKER, Shayne T. Cause of Ogden, Utah Action: Respondent has been practicing as a nurse in Utah without a Utah
license. Memorandum of Understanding and Order: A license to practice as a licensed practical nurse was issued.
Respondent was also ordered to pay a $500 fine within 120 days
and was given a public reprimand for practicing without an
appropriate license. Date: December 15, 2005 Newsletter - January 1, 2006 - Page 8
PHARMACISTS/PHARMACY TECHNICIANS/PHARMACIES
ABRAMS, David B. Cause of Lehi, Utah Action: Respondent admitted taking controlled substances from an
employer without having a legal prescription for the drugs in his
possession at the time of dispensing. Respondent stated he took
the controlled substances for his wife's use. Order: Respondent's licenses to practice as a pharmacist and to dispense
controlled substances were revoked with a stay of enforcement on
the revocation. Said licenses were placed on probation for three
years with conditions and restrictions. Date: November 10, 2005
BLACK, Kristy Ann Reinstatement Pharmacy Technician Order: The probation on Respondent's license to practice as a pharmacy Sandy, Utah technician was terminated and said license was reinstated with full
privileges. Date: December 22, 2005
BOWCUTT, Timothy Amended Tremonton, Utah Order: The probationary condition requiring Respondent to work as a
pharmacist under "direct supervision", which was contained in his
March 24, 2003 practice plan, was changed to allow Respondent to
work as a pharmacist under general supervision. This amendment
was made retroactive to September 27, 2005. Date: November 3, 2005
BUCKLEY, Andrew Howell Amended Farmington, Utah Order: The probationary condition requiring Respondent to participate in
therapy and/or aftercare was terminated. Date: October 27, 2005
JOLLEY'S CORNER PHARMACY Cause of Class A pharmacy Action: A non-licensed employee in the pharmacy diverted controlled Sandy, Utah substances from the pharmacy stock after regular pharmacy hours
for her own personal use. Order: Respondent's licenses to practice as a pharmacy and to dispense
controlled substances were publicly reprimanded for neglecting to
maintain controls over controlled substances against diversion,
theft, or shortage of controlled substances as outlined in the Utah
Controlled Substance Act Rules. Respondent was ordered to pay a
$2,000 fine within 30 days. Respondent's owner is also required to
meet with the Pharmacy Board within 30 days. Date: October 26, 2005
KOHLER, Susan B. Cause of Pharmacy Technician Action: Respondent failed to comply with probationary conditions that Heber City, Utah existed on her license. Order: The stay of suspension that was entered on Respondent's license
in a June 21, 2005 Default Order was vacated and the suspension
of Respondent's license to practice as a pharmacy technician
became effective the date of the Order. Date: October 31, 2005 KRUGER, Cheryl Eileen Cause of Pharmacy Technician Action: Respondent has a controlled substance abuse problem Salt Lake City, Utah Order: Respondent voluntarily surrendered her license to practice as a
pharmacy technician. Respondent may not reapply for licensure for

MEYER, Eugene L. Reinstatement Blanding, Utah Order: The probation on Respondent's licenses to practice as a pharmacist and to dispense controlled substances was terminated and said
licenses were reinstated with full privileges retroactive to May 24,
2005. Date: November 15, 2005
MILLIKAN, Diann M. Amended Salt Lake City, Utah Order: The suspension on Respondent's license to practice as a
pharmacist was terminated and said license was placed on
probation for five years subject to the conditions and restrictions
identified in the August 22, 2005 Stipulation and Order. It should be
noted that Respondent's license to dispense controlled substances
will remain suspended at this time. Date: October 13, 2005
PARK CITY PHARMACY @ THE CLINIC
Class A pharmacy Emergency Park City, Utah Order: The licenses of Park City Pharmacy @ the Clinic to practice as a
Class A pharmacy and as a dispensary of controlled substances
were also suspended given the fact that the pharmacy's
pharmacist-in-charge, Thomas Evan Strebel, currently has
suspended licenses. The suspension will remain on the pharmacy
licenses until further review and approval by the Division of a new
pharmacist-in-charge. Date: October 31, 2005
PRECISION RX, INC. Cause of Class D pharmacy Action: In February 2004 Respondent was disciplined by the Texas State Fort Worth, Texas Board of Pharmacy. Order: Respondent was publicly reprimanded and Respondent's
pharmacist-in-charge is to meet with the Utah State Board of
Pharmacy if so requested. Date: October 18, 2005
REMUND, Wesley Charles Cause of Lehi, Utah Action: Respondent engaged in unprofessional conduct with respect to the
misuse of drugs. Order: Respondent was ordered to pay a $1,000 fine within 30 days.
Respondent's licenses to practice as a pharmacist and to dispense
controlled substances were revoked with a stay of enforcement on
the revocation. Said licenses are suspended until such time as
Respondent completes certain conditions outlined in the Order.
Following the period of suspension, said licenses will be placed on
probation for five years with conditions and restrictions. Date: October 18, 2005 Amended
Order: The suspension on Respondent's licenses to practice as a
pharmacist and to dispense controlled substances was terminated
and said licenses were placed on probation for five years subject to
the conditions and restrictions identified in the October 18, 2005
Stipulation and Order. Date: October 27, 2005
STEWART, Scott Edward Amended Draper, Utah Order: The probationary condition requiring performance evaluations from
Respondent's employer on a monthly basis was changed to require

STREBEL, Thomas Evan Cause of Park City, Utah Action: The Division determined that the continued practice of Respondent

controlled substances were immediately suspended pending a
hearing and a further Division Order being issued. Date: October 31, 2005
WRAY, Kirtland J. Cause of Woods Cross, Utah Action: Respondent admits he has a substance abuse problem that might
reasonably be considered to impair his ability to safely engage in
the practice of pharmacy. Order: Respondent voluntarily surrendered his licenses to practice as a
pharmacist and to dispense controlled substances. Respondent
may not reapply for licensure for a period of one year for his
pharmacist license and for a period of three years for his dispensing
controlled substance license. Date: November 22, 2005
CONTRACTORS/UNLICENSED CONTRACTORS
HOFF CONSTRUCTION, LLC Cause of HOFF, Michael Leon Action: Respondent engaged in unprofessional conduct prior to submitting Syracuse, Utah an application for licensure. Memorandum of Understanding and Order: A license to practice as a general building contractor was issued
and placed on probation for four years with conditions. Date: November 30, 2005
OTHER REGULATED PROFESSIONS
ALLGIER, Michael Anne Reinstatement Licensed Substance Abuse Counselor Order: The probation on Respondent's license to practice as a licensed Stansbury Park, Utah substance abuse counselor was terminated and said license was
reinstated with full privileges. Date: November 10, 2005
ANDREWS, Thomas Earl Cause of Certified Public Accountant Action: Respondent has been convicted of crimes which reflect moral Nephi, Utah turpitude and which, when considered with the functions and duties
of a certified public accountant, bear a reasonable relationship to
his ability to safely and competently practice his profession. Order: Respondent's license to practice as a certified public accountant

OTHER REGULATED PROFESSIONS - continued
BAUM, Brandy Marie Cause of Massage Therapist Action: Respondent engaged in unprofessional conduct prior to submitting Provo, Utah an application for licensure. Memorandum of Understanding and Order: A temporary massage therapist license was issued and placed on
probation for two years with conditions and restrictions. Respondent must submit evidence to the Division of having passed
the national massage examination within 120 days or the temporary
license will expire. If Respondent successfully passes the massage
examination, a probationary massage therapist license will be
issued with conditions and restrictions. Date: December 1, 2005
BJORNSON, Michael Jonathan Cause of Psychologist Action: Respondent submitted false information to a company in two Vernal, Utah instances through his application to renew provider status regarding
disciplinary action against his license. Order: Respondent's license to practice as a psychologist was revoked
with a stay of enforcement on the revocation. Said license will be
suspended for two (2) months effective thirty (30) days from the
date of the Order (November 2, 2005). Following the period of
suspension, said license will revert to probationary status for six
years subject to the conditions and restrictions that currently exist
on his license and additional conditions. Date: October 3, 2005 Amended
Order: Respondent was granted an additional 12 months to complete the
required 24 hours of continuing professional education ordered by
the Division in its October 3, 2005. Date: November 15, 2005
BUONOCORE, Marylynn Cause of Unlicensed Medicine Action: Respondent worked as a technician utilizing a laser medical device South Jordan, Utah without the direct supervision of a physician. Order: Respondent was ordered to cease and desist from engaging in the
unlicensed practice of medicine until she is properly licensed to do
so. Date: December 1, 2005
BRAUN, David Anthony Cause of Chiropractic Physician Action: Respondent's license to practice as a chiropractic physician was Salt Lake City, Utah disciplined by the North Dakota Board prior to Respondent applying

a stay of enforcement on the revocation. Said license was placed
on probation for one year with conditions. Respondent shall cease

ANDREW, Larry Glen, DO Cause of Springville, Utah Action: Respondent's continued practice represents an immediate and
significant danger to the public health, safety and welfare. Emergency
Order: Respondent's licenses to practice as an osteopathic
physician/surgeon and to administer and prescribe controlled
substances were immediately suspended pending a further order of
the Division. Date: December 23, 2005
BAILEY, Stanton Anderson, MD Amended Santaquin, Utah Order: The probationary condition requiring Respondent to attend 12-step
program meetings at least five (5) times per week was changed to
require Respondent to attend a minimum of four (4) meetings per
week. Date: November 3, 2005
BERK, Howard Sidney, MD Cause of Murray, Utah Action: Respondent engaged in behavior in July 2005 that may adversely
reflect on his ability to safely and competently practice as a
physician/surgeon. Order: Respondent's licenses to practice as a physician/surgeon and to
administer and prescribe controlled substances are temporarily
restricted to the non-clinical practice of medicine. Respondent shall
also restrict his practice to: (a) paper review of care given by other
medical practitioners, (b) research, and (c) education. Respondent
shall not engage in direct patient contact, interview, examination,

CLARK, Jeffery Allen, MD Cause of Provo, Utah Action: Respondent has had a lengthy history of substance abuse problems
and controlled substance violations. Order: Respondent's license to administer and prescribe controlled
substances was revoked effective the date of the Order.
Respondent's license to practice as a physician/surgeon was
revoked with a stay of enforcement on the revocation. Said
physician/surgeon license is suspended until certain conditions
have been met. Following the period of suspension, said license

PROSKAUER, Stephen, MD Reinstatement Salt Lake City, Utah Order: The probation on Respondent's licenses to practice as a
physician/surgeon and to administer and prescribe controlled
substances was terminated and said licenses were reinstated with

WEBER, Nita Beth, DO Amended Nephi, Utah Order: The suspension on Respondent's license to administer and
prescribe controlled substances was terminated and said license
was placed on probation subject to the same probationary
conditions that currently exist on her license to practice as an

DENTISTS
FIFIELD, Perry Melvin Cause of Clearfield, Utah Action: In October 2003 and April 2004 Respondent obtained and provided
protected information belonging to D.W. contained in the Utah
Controlled Substance Database to his employer who was involved
in an ongoing domestic dispute with D.W. In April 2003
Respondent was disciplined by the California Dental Board. When
Respondent renewed his Utah dental licenses in May 2004, he
failed to notify the Division of the disciplinary action taken in
California. Order: Respondent's licenses to practice as a dentist and to administer
and prescribe controlled substances are revoked with a stay of
enforcement on the revocation. Said licenses are placed on

FORD, Kraig R. Cause of Heber City, Utah Action: Respondent failed to make sure controls were in place regarding
the maintenance of his office's nitrous oxide supply. Respondent
also had previously prescribed a Schedule III controlled substance
to a patient for lower gastrointestinal problems, a condition which
Respondent is not licensed to treat. Respondent also failed to
properly document controlled substance prescriptions issued to
patients in patient charts on numerous occasions. Order: Respondent's license to practice as a dentist with a Class II
anesthesia permit was revoked. A license to practice as a dentist
with a Class I anesthesia was issued to Respondent. Respondent's
licenses to practice as a dentist with a Class I anesthesia permit
and to administer and prescribe controlled substances were
revoked with a stay of enforcement on the revocation. Said licenses
were placed on probation for two years with conditions. Respondent was also ordered to pay a $1,000 fine within 90 days. Date: October 26, 2005
REGISTERED NURSES/ADVANCED PRACTICE NURSES
BAIRD, Jamie Steadman Cause of Tooele, Utah Action: Due to poor health, Respondent is not able to comply with
probationary conditions that exist on her license. Order: Respondent's license to practice as a registered nurse is indefinitely
suspended. Respondent may apply for the termination of the
suspension at any time upon compliance with certain conditions
before the suspension can be terminated. Date: October 18, 2005
BUDGE, Jennifer Lea Cause of Monticello, Utah Action: Respondent did not feel she is able to comply with terms and
conditions that exist on her license. Order: Respondent voluntarily surrendered her license to practice as a
registered nurse. Respondent may not reapply for licensure for a
period of three years. Date: November 14, 2005
CHIPMAN, Brenda Amended American Fork, Utah Order: The probationary condition requiring Respondent to consult with
only one physician with regards to prescriptions was changed to
allow Respondent's primary care physician, Dr. Marshall Willis, to
prescribe all mood altering or narcotic medications and to allow
other practitioners in Dr. Willis practice to prescription other
medications to Respondent. Date: October 27, 2005
COX, Tiffany Ann Cause of Heber, Utah Action: Respondent's license to practice as a registered nurse expired on
January 31, 2005 yet Respondent continued to practice as a nurse. Order: Respondent was publicly reprimanded and ordered to pay a $500

ROONEY, Ann Cause of Salt Lake City, Utah Action: Respondent has engaged in conduct, including the use of
intoxicants and controlled substances that might reasonably be
considered to impair her ability to safely engage in the practice of
nursing. Order: Respondent's license to practice as a registered nurse was revoked
with a stay of enforcement on the revocation. Said license was
placed on probation for five years with conditions and restrictions. Date: November 3, 2005
SALAS, Glenda J. Amended North Ogden, Utah Order: The probationary condition restricting Respondent's access to
controlled substances was terminated. Date: December 19, 2005 STEINER, Richard Alan Cause of St. George, Utah Action: Respondent has a substance abuse problem that, if left untreated,
renders him unsafe to practice nursing. Respondent took numerous
units of a Schedule III controlled substance form his employer
without authorization and without a legal prescription. Respondent
later ingested the controlled substances he took. Order: Respondent was ordered to pay a $1,000 fine within 90 days.
Respondent's license to practice as a registered nurse was revoked
with a stay of enforcement on the revocation. Said license was

STORMS, Dawna P. Cause of Salt Lake City, Utah Action: Respondent has a substance abuse problem that, if left untreated,
may render her unfit to practice nursing. Order: Respondent was ordered to pay a $500 fine. Respondent's license
to practice as a registered nurse was revoked with a stay of
enforcement on the revocation. Said license is suspended until
such time as certain conditions have been completed. Following
the period of suspension, said license will be placed on probation
for five years with conditions and restrictions. Date: December 13, 2005
WHITEBREAD, Teresa L. Cause of Riverton, Utah Action: Respondent has an alcohol abuse problem that might reasonably
be considered to impair her ability to safely engage in the practice
of nursing. Order: Respondent's license to practice as a registered nurse was revoked
with a stay of enforcement on the revocation. Said license was
placed on probation for five years with conditions and restrictions. Date: November 16, 2005 Amended
Order: The probationary condition restricting Respondent's access to
controlled substances was amended to allow Respondent to have
access to controlled substances only while employed in the labor
and delivery section at IHC central urban facilities if she is actively
participating in the Employee Assistance Program. Date: December 22, 2005
WILLDEN, Virginia B. Cause of Tremonton, Utah Action: In May 2005 Respondent knowingly falsified blood sugar levels in
the record of a diabetic patient and failed to document several of the
patient's blood sugar readings. Order: Respondent was given a public reprimand and ordered to pay a
$500 fine within 90 days. Date: November 30, 2005
WILLIAMS, Teresa Yonk Reinstatement Hyrum, Utah Order: The probation on Respondent's license to practice as a registered
nurse was terminated and said license was reinstated with full
privileges. Date: October 6, 2005

KNOWLES, Linda Cause of Layton, Utah Action: While employed as a LPN, Respondent conspired with a nursing
aide to provide the aide with a Schedule III controlled substance.
The nursing aide would sell the controlled substance and return the
proceeds to Respondent. Respondent also provided the aide with
the controlled substance for the aide's personal use. Order: Respondent voluntarily surrendered her license to practice as a
licensed practical nurse. Respondent may not reapply for licensure
for a period of five years. Date: November 8, 2005
KOELLER, Bambi Bodenhofer Amended Midvale, Utah Order: The suspension on Respondent's license to practice as a licensed
practical nurse was terminated and said license was placed on
probation for five years subject to conditions and restrictions
identified in the Division's September 22, 2003 Memorandum of
Understanding and the December 22, 2005 Amended Order. Date: December 22, 2005
MARTINEZ, Barbara A. Cause of Salt Lake City, Utah Action: Respondent's license to practice as a licensed practical nurse
expired on January 31, 2004 yet Respondent continued to work as
a nurse until October 2005. Order: Respondent was publicly reprimanded and ordered to pay a $500
fine within 90 days of the Order date. Date: November 18, 2005
WALKER, Shayne T. Cause of Ogden, Utah Action: Respondent has been practicing as a nurse in Utah without a Utah
license. Memorandum of Understanding and Order: A license to practice as a licensed practical nurse was issued.
Respondent was also ordered to pay a $500 fine within 120 days
and was given a public reprimand for practicing without an

ABRAMS, David B. Cause of Lehi, Utah Action: Respondent admitted taking controlled substances from an
employer without having a legal prescription for the drugs in his
possession at the time of dispensing. Respondent stated he took
the controlled substances for his wife's use. Order: Respondent's licenses to practice as a pharmacist and to dispense
controlled substances were revoked with a stay of enforcement on
the revocation. Said licenses were placed on probation for three
years with conditions and restrictions. Date: November 10, 2005
BLACK, Kristy Ann Reinstatement Pharmacy Technician Order: The probation on Respondent's license to practice as a pharmacy Sandy, Utah technician was terminated and said license was reinstated with full

BOWCUTT, Timothy Amended Tremonton, Utah Order: The probationary condition requiring Respondent to work as a
pharmacist under "direct supervision", which was contained in his
March 24, 2003 practice plan, was changed to allow Respondent to
work as a pharmacist under general supervision. This amendment
was made retroactive to September 27, 2005. Date: November 3, 2005
BUCKLEY, Andrew Howell Amended Farmington, Utah Order: The probationary condition requiring Respondent to participate in
therapy and/or aftercare was terminated. Date: October 27, 2005
JOLLEY'S CORNER PHARMACY Cause of Class A pharmacy Action: A non-licensed employee in the pharmacy diverted controlled Sandy, Utah substances from the pharmacy stock after regular pharmacy hours
for her own personal use. Order: Respondent's licenses to practice as a pharmacy and to dispense
controlled substances were publicly reprimanded for neglecting to
maintain controls over controlled substances against diversion,
theft, or shortage of controlled substances as outlined in the Utah
Controlled Substance Act Rules. Respondent was ordered to pay a
$2,000 fine within 30 days. Respondent's owner is also required to
meet with the Pharmacy Board within 30 days. Date: October 26, 2005
KOHLER, Susan B. Cause of Pharmacy Technician Action: Respondent failed to comply with probationary conditions that Heber City, Utah existed on her license. Order: The stay of suspension that was entered on Respondent's license
in a June 21, 2005 Default Order was vacated and the suspension
of Respondent's license to practice as a pharmacy technician
became effective the date of the Order. Date: October 31, 2005 KRUGER, Cheryl Eileen Cause of Pharmacy Technician Action: Respondent has a controlled substance abuse problem Salt Lake City, Utah Order: Respondent voluntarily surrendered her license to practice as a
