ShipTo: Attention: Start Date: Completion Date (Est):With this agreement you accept responsibility for all rental charges, start dates, finish dates, freight to and from factory and missing/damaged parts per Asahi/America's Proweld Rental Program Contract. A copy of this contract is on record at your office. A copy will be provided upon request.
All billing begins when the tool arrives at jobsite. Billing stops once an RMA# is requested through Janice in our Rental Dept. Please requst an RMA BEFORE returning rental tools. Please ship tools back FREIGHT PREPAID.
Tools not returned within 7-10 days of the RMA issue date will be billed in full until receipt of equipment.
If you have any concerns with regards to your obligations, please do not hesitate to contact us.
Asahi/America will release the equipment for shipment only upon receipt of this signed agreement. Please sign and FAX to: 781-388-4749, Attn: Dave Hume.
