Benefit Employee Cost When Eligible Benefit Description Health Coverage Prescriptions Mail-order prescriptions Self-insured - See  Policy Co-pay $10 generic $16 brand name $10/90 day supply generic $20/90 day supply brand name Completion of 90 days (Coverage available from first day of employment if transferring directly from another denom. employer. ) Medical, dental and vision coverage is available to full-time employees.  Please refer to the Health Care Assistance Plan document for details. www.adventistrisk.org/employee benefits/healthcare.html Short Term Sick Leave/Extended Term Sick Leave Employer provided plan 90 days Sick leave of 2 weeks per year (60% for short-term and 40% for long-term sick leave).  Extended term sick leave can be accumulated up to a maximum of 1,000 hours. Vacation Employer provided plan 90 days Vacation is based on accrual basis and determined by years of denominational service.  Up to 4 weeks/year available. Accrual caps at 240 hours.  Faculty on 95% plan do not accrue vacation in lieu of having summers and breaks off Holiday Pay Employer provided Immediate Nine holidays; regular part-time employees receive pro-rated pay.  Does not apply to faculty on 95% pay plan Employee Basic Life Coverage Employer provided plan Immediate Death benefits paid directly to named beneficiary.  Coverage amounts $100,000 employee; $50,000 spouse; $5,000 dependents Supplemental Life Insurance (Hartford) Actual, based on benefit purchased Immediately or per policy Coverage may be purchased in increments of $10,000 up to $750,000 not to exceed 7 times earnings.  You may purchase an equal coverage amount for spouse not to exceed 100% of your coverage amount.  Coverage for dependents not to exceed $25,000 AD&D (Hartford) Optional Accidental Death & Dismemberment Varies, based on employee purchase Immediate (See Business Office for details) Coverage may be purchased in increments of $10,000 up to $500,000, not to exceed 10 times earnings.  You may purchase an equal coverage amount for spouse not to exceed 100% of your coverage amount.  Coverage for dependents up to $25,000 not to exceed 100% of your coverage amount. Long Term Disability (Hartford) Employer paid Must meet policy requirements.  (Eligibility begins 91st day) A.  Salary continuation (66 2/3%).      Ability Plus (in addition to basic      LTD benefits) additional 20% of      your basic monthly earnings to a      max of $6,000. B.   Spouse Disability ($1,500 for 24      months after a 60-day elimination      period) (taxable income) Social Security As required by law Immediate Financial assistance at retirement, death or if disabled.  Regular, part-time employees. Adventist Retirement Plan/VALIC Per policy 3 year cliff-vesting provision for employer contributions Defined Contribution Plan.  Employer contributes 5% of gross wage.  If employee contributes 2.50% of gross wage, employer contributes additional 1.25% Supplemental Benefit Plans (AFLAC) Varies, based on benefits purchased Must meet policy requirements (open enrollment - December 1-31 annually) a) Cancer Insurance b) Accident Insurance c) Hospital Intensive Care Workers' Compensation Immediate Indemnity for loss of wages at 2/3 gross wage and payment of medical expenses resulting from on-the-job injuries/ illnesses as prescribed by state law. Tax Deferred Annuity (VALIC or Lincoln Life) Varies, based on employee choices Immediate Designed to assist employees with retirement income and is done through payroll deduction Moving Expenses Employer paid - per policy Immediate All approved moving expenses (within policy limits) are paid for individuals asked to join Union College through relocation from another state Free UC Tuition - Employee/Spouse Employer paid Immediate Employee working at least 20 hours per week may receive up to 4 hours of free class tuition, as space is available. Benefit can be transferred to spouse. Tuition Allowance - Children Employer paid See Business Office for details regarding eligibility and waiting period for part- time employees 70% of tuition for dorm students 35% of tuition if living at home Union reserves the right to make changes in any provision or requirement at any time.
