Steps for Making the Supervisory Referral  

The Employee Assistance Program (EAP) works at TWO basic levels: 

SELF-REFERRAL   - Employee calls directly.

                                          No supervisory involvement.

SUPERVISORY REFERRAL  - Employee referred by

                                          supervisor because of JOB

                                          PERFORMANCE problems. 

Steps for making the SUPERVISORY REFERRAL: 

1. Put in writing!  Document specific work performance problems.  Write down dates, incidents and job performance deficits. 

2. Confront AND support the employee.  Be realistic with the employee about his/her failing performance.  Convey your concern for him/her as a VALUED employee. 

3. Inform employee of availability of the EAP and how to utilize this confidential service.  Advise that this is a suggestion to seek help on his/her own. 

4. If performance problems persist, follow steps 1 & 2 above and MAKE THE REFERRAL:

a. Call 716-5493 and ask for an EAP counselor.

b. Explain WORK PERFORMANCE situation to the counselor.

c. Write down the employee's appointment time and place.  Keep a record of the appointment for yourself.

d. Give appointment time to the employee.

e. Emphasize importance of keeping the appointment.

f. Follow-up.  Check with the employee AND the EAP counselor to see if the appointment is kept. 

5. Follow-up.  After the EAP referral, continue to supervise job performance.  Document problems and confront the employee honestly.  Reinforce his/her good points.  Be consistent.  Don't over supervise.
